During COVID19, please contact the Business Office Manager at 410-221-2266 to do an intake over the phone. You will need to have your insurance information available. You will then need to complete the new client paperwork, which is available to download from our website or can be picked up from our office. Please include a copy of your state issued ID card and insurance card. For minors, a copy of the birth certificate and/or custody agreement is also required. Once the office receives your completed paperwork, they will call you to schedule an initial evaluation with a therapist.
Our services are approved for Medicaid and Medicare reimbursement and many private insurance plans, including BlueCross/BlueShield, Magellan, United Behavioral Health, Aetna, Claimsbridge, and Cigna. Our clinic offers a sliding scale for payment based on income level and eligibility. If you do not see your insurance provider listed, please contact the Business Office to inquire.
We provide an interactive patient portal through our Electronic Health Records system, AdvancedMD. If you have a patient portal account, follow this link to access the portal. For information on setting up a portal account, please read this document. If you need further help accessing your portal account, please contact our office.
